Do you have new Contact Details?
Do you have a mobile number or email address? Have you changed your address or telephone numbers recently?

If you provide your mobile number we may send you text messages regarding certain services. Text messages will not normally be used to remind you of booked appointments.

Please contact us with your new details, or fill out our online form so we can ensure our details are accurate, thank you.

To Register as a New Patient
Please contact Reception to register as a new patient. We will ask you to complete a registration form.

Please bring the following to the practice:

  • Photographic ID
  • Your registration form
  • Your NHS number (if available)
  • Your repeat medication list
  • Details of your previous doctor

Please note patients are registered with the practice rather than with an individual doctor. You may wish to request services with a particular doctor. Please state your preference on the registration form.

We will offer you an opportunity to see the Healthcare Assistant for a new patient check, or to discuss medication. If you have a complicated medical history it may be appropriate to have a further appointment with the doctor.

May we take this opportunity to welcome you to our Practice and to invite you to complete our quick Patient Questionnaire. This will give us some additional details which will allow us to provide the most appropriate services for you.

Repeat Prescriptions
If you are on a regular treatment, please request repeat prescriptions online or by ticking all items you require on the slip issued with your prescription. Call in or send in by post.

Please allow 48 hours for your prescription to be processed. If possible, order all items required for the following month.

You may collect from Reception or the Pharmacy of your choice as prescriptions can now be sent to any Pharmacy electronically.

Would you like to collect your prescriptions without requesting them each month?
Depending on your medication, we may be able to authorise several months of prescriptions in advance. You can then collect your medication from the pharmacy each month without having to request it each time from the Practice. However, please make sure that you request further prescription forms when you collect the last prescription of your batch.

Practice Area
Click here to view our practice area. We welcome new patients who live or have moved into this area. If you are an existing patient who has moved slightly out of our practice area, you may continue to be registered (at the discretion of the practice), if you still live within our outer practice area. It is in the interests of patients who require regular home visits to choose a practice within a reasonable travelling distance of where they live.

Confidentiality
We take confidentiality seriously and all staff are bound by the same rules of confidentiality as the doctors. Please give Reception as much information as possible which will enable us to get you an appointment sooner with the most appropriate clinician.

Access to Premises
The surgery has disabled access and toilet facilities. Our patient services are situated on the ground floor. There is level access throughout, including from the patient and disabled parking areas to Reception. Disabled parking is available near the entrance. There is free parking in the car park at the rear of the building or on the main road at the front. Other parking is situated at the bus turnaround near the school.

Patient Rights and Responsibilities
You have a right to:

  • Full Confidentiality in respect of all aspects of your personal health information and medical history.
  • Courtesy will be shown at all times from Doctors, Practice Staff and other Health Service Staff.
  • Information – you have the right to ask questions about your own health and have your questions answered. The Practice allows you access to your record on request, in line with current legislation.
  • Smoke Free Environment – the Practice operates a no-smoking policy.

The Practice does not discriminate on grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

Help Us to Help You
You are responsible as follows:

  • Please keep your appointment or give adequate notice that you wish to cancel it.
  • Please do not delay the doctor - please show consideration to other patients. Remember an appointment is for 10 minutes and for one person only.
  • Please come to the surgery when not prevented by serious illness or infirmity. A doctor can see more patients in the surgery than when out visiting.
  • Please be responsible for your own health – take appropriate action to prevent ill health, by following advice given by Health professionals.
  • Zero tolerance to Violence – where a patient is violent or abusive to persons or premises, the practice will take action to withdraw services from this patient.
